COMO IDENTIFICAR NEUROMAS TRAUMÁTICOS CERVICAIS NO PÓS-OPERATÓRIO DE ESVAZIAMENTO DE PESCOÇO: REVISÃO BREVE DO ULTRASSOM DO PLEXO CERVICAL
HOW TO IDENTIFY CERVICAL TRAUMATIC NEUROMAS IN THE POST-OPERATIVE NECK DISSECTION: BRIEF REVIEW OF THE CERVICAL PLEXUS ULTRASOUND

Resumo
Os neuromas traumáticos cervicais (NTCs) correspondem a uma hiperplasia não neoplásica de nervos lesionados e podem ser erroneamente interpretados como linfonodos metastáticos durante o exame ultrassonográfico cervical de pacientes com câncer de tireoide submetidos ao esvaziamento cervical lateral, resultando em punções aspirativas por agulha fina desnecessárias e dolorosas. A anatomia do plexo cervical é complexa, e a identificação correta de suas raízes e ramos nervosos ao ultrassom é fundamental para o adequado diagnóstico dos NTCs. Esta revisão descreve a técnica ultrassonográfica utilizada para caracterizar NTCs em 10 etapas padronizadas e apresenta os principais aspectos sonográficos do plexo cervical, auxiliando o radiologista na diferenciação entre NTCs e linfonodos metastáticos.
Abstract
Cervical traumatic neuromas (CTNs) are a non-neoplastic hyperplasia of damaged nerves and may be misinterpreted as metastatic lymph nodes during cervical ultrasound of patients with thyroid cancer after lateral neck dissection, resulting in unnecessary and painful fin-needle aspiration biopsy. The anatomy of the cervical plexus is challenging, and the correct identification and recognition of its neural roots and branches on ultra-sound are key to the correct CTN diagnosis. This review outlines the US technique used to characterize CTNs in 10 standardized steps and describes the sonographic features of the cervical plexus, to assist the radiologist in the recognition of CTNs and their differentiation from lymph node metastases.

Resumo
Terapias crioablativas têm sido propostas para o controle paliativo da dor em tumores de partes moles ou lesões ósseas osteolíticas. Este relato descreve o caso de um paciente com metástases escleróticas dolorosas na coluna torácica e sacral, tratado com sucesso por crioablação percutânea guiada por imagem, utilizando técnicas de isolamento térmico e termosensores.
Abstract
Cryoablative therapies have been proposed to palliate pain from soft-tissue or osteolytic bone tumors.
A case of a patient with painful thoracic and sacral spine sclerotic metastases successfully treated by image-guided percutaneous cryoablation with the aid of insulation techniques and thermosensors is reported in this case report.

Resumo
A ablação a laser guiada por ultrassom tem emergido como alternativa minimamente invasiva para o tratamento de nódulos tireoidianos benignos (NTs), especialmente em pacientes que buscam evitar cirurgia ou apresentam contraindicações ao tratamento convencional. Considerando que a quantidade de energia aplicada pode influenciar os desfechos clínicos e volumétricos, este estudo avaliou a eficácia da técnica sob diferentes parâmetros energéticos. Foram analisados 34 pacientes eutireoideos com NTs entre 5–18 ml, tratados com ablação a laser ou acompanhados clinicamente por até 12 meses.
Abstract
Ultrasound-guided laser ablation has emerged as a minimally invasive alternative for the treatment of benign thyroid nodules (TNs), particularly in patients seeking to avoid surgery or who present contraindications to conventional management. Considering that the amount of delivered energy may influence both clinical and volumetric outcomes, this study evaluated the effectiveness of the technique under different energy parameters. A total of 34 euthyroid patients with TNs ranging from 5–18 ml were analyzed, either treated with laser ablation or clinically monitored for up to 12 months.
